Family-owned and spiritually-based, Alinea Family Hospice Care offers patients and their families kind, compassionate, and innovative hospice care. We serve patients out of three offices in Terrell, Athens, and Waxahachie, TX, and our service areas stretch across 22 North and East Texas counties. Our services are designed to provide individuals and their families with comfort, hope, and support throughout every stage of care.

Hospice generally does not seek a cure, but enriches life without hastening or postponing one’s end of life transition. At Alinea Family Hospice Care, we recognize the importance of nurturing the physical, emotional, and spiritual needs of the patient and their families. By focusing on the person’s needs and managing the symptoms of a disease — and highlighting quality of life versus length of life — we are able to fully enrich their lives and prepare them for the final event of life.
Our work is a celebration of life and is designed to provide great support and guidance in making many important decisions involving both the patient and their family.
Meet Our Medical Directors & Physicians
Matthew Glick, DO- Medical Director
Dr. Glick is “Board Certified” by the American Board of Family Practitioners and is licensed to practice medicine in the State of Texas. He has a busy practice in Kaufman Texas and is affiliated with the Presbyterian Hospital of Kaufman where he serves as “Director of Palliative Care Services”. Dr. Glick is well known in the community for his many volunteer services, including St. Luke Health Ministry, a clinic for Indigent health care. He has been awarded Humanitarian of the year by the Kaufman Chamber of Commerce.
Dr. Les Sandknop, DO- Alternate Medical Director
Dr. Les Sandknop, DO, is a Family Practice specialist in Rockwall, Texas. He is a graduate Kirksville College of Osteopathic Medicine. He is affiliated with several hospitals including Baylor Scott and White in Dallas and Rowlett, Texas Health Presbyterian Hospital in Rockwall, Hunt Regional Medical Center in Greenville. Dr. Les Sandknop also cooperates with other doctors and physicians in medical groups, including Greater Dallas Healthcare Enterprises. He is well known in the community with 39 years of diverse experience and is a specialist in Family Practice. Dr. Sandknop is well known for his compassion for his patients and dedicated efforts to provide specialized care for their well-being.
Dr. Patrick Boothe, DO- Regional Medical Director North
Dr. Boothe is a board certified family physician with over 15 years of experience in family medicine and hospice care. He obtained a bachelor of science degree in electrical and biomedical engineering from Southern Methodist University. After completing medical school at the University of North Texas Health Science Center, he went on to complete his medical training and board certification at the Methodist Family Medicine Residency Program in Dallas, TX, serving as chief resident in his final year. He has been a dedicated physician to North Texas since that time. In addition to his commitment to family medicine and hospice care he has served as a medical officer in the United States Navy during the entirety of his medical career. He served on active duty in Afghanistan in 2012, and he continues to serve as a reserve medical officer in the Navy. Dr. Boothe prides himself in providing compassionate, exceptional, and personalized care.

Meet Our Owners
Donna Junkersfeld – Owner/Administrator
Donna is currently both the owner and clinical administrator for Alinea Family Hospice Care in Terrell, Tx. She had been a licensed RN for the State of Texas since 1997 and has been involved with various levels of the healthcare sector for the last 30 years including Long Term Care, Med Surge, Home Health, and ultimately Hospice Care. Donna brings a wealth of knowledge, not only in providing excellent patient care, but in the management of the regulations, policies, and procedures that are required to provide compliant care in conjunction with the most current Medicare and Medicaid state and federal hospice laws.
Recently, she has developed a greater passion for transitions of hospice care as a family member of a patient. It was a great learning experience that has helped her relate more to what the patient and their families go through when they are under our service. Donna’s personal and professional experiences have been the driving factors behind the great passion of providing the absolute best care and comfort for patients.
She is blessed with having an incredible staff to lead at Alinea Family Hospice Care and has a passion for being a great leader and trying to inspire the team to be better today than they were yesterday. She truly believes that learning and growing every day is a requirement, not an option.
Donna is married and is both a mother and a grandmother. She has lived the majority of her life in the East Texas area currently residing in Wills Point, Texas. She enjoys spending time with her family, church activities, and working in her garden. She believes that her path is to “Serve God through Serving People!”
Bryan “Tracy” Craft – Owner & Chief Operating Officer
Bryan “Tracy” Craft serves as Owner and Chief Operating Officer of Alinea Family Hospice Care in Terrell, Texas. With more than three decades of distinguished military service and leadership experience, Tracy brings a disciplined, mission-driven approach to operations, regulatory compliance, and compassionate care.
A graduate of Edgewood High School, Tracy continued his education at Tyler Junior College, earning his degree through the Air Force. In 1986, he joined the Air National Guard, and in 1989 transitioned to full-time service as an Air Reserve Technician with the United States Air Force.
Throughout his 33½-year military career, Tracy was stationed at Carswell Air Force Base, serving the United States with honor and dedication. His service included multiple deployments across Southeast Asia, Europe, and South America. In 2014, he was promoted to the prestigious rank of E-9 Chief Master Sergeant, where he oversaw and managed critical processes and procedures related to air safety and avionics operations. His leadership required exceptional attention to operational efficiency, regulatory adherence, and mission readiness—skills that now directly support his work in healthcare operations.
After retiring from the Air Force in 2018, Tracy felt called to invest his leadership experience into a more personal and community-centered mission. He developed a deep passion for hospice care and now serves as COO of Alinea Family Hospice Care, where he focuses on operational excellence while ensuring that patient dignity, family support, and compassionate service remain the heart of the organization.
Tracy believes strongly that integrity must guide every decision. He often reflects on the principle: “One must do what they are supposed to do before doing what they want to do.” In the inherently high-stress environment of hospice care, he emphasizes doing the right thing first—always treating patients, families, and staff with urgency, compassion, and empathy. Under his leadership, Alinea Family Hospice Care continues to uphold the highest standards of compliance, professionalism, and heartfelt service.
Tracy married his high school sweetheart, Shelly Craft, in 1991. A lifelong resident of East Texas, he currently resides in Edgewood, Texas. He enjoys spending time with his family, participating in church activities, playing golf, traveling, and caring for his many animals. He considers it both a privilege and a blessing to serve families during one of life’s most sacred seasons and believes hospice care provides immeasurable value, comfort, and peace.

Our devoted team of nurses and staff are available at all times, 24 hours a day, 365 days a year, and are all highly experienced in managing patient comfort. Our registered nurses assess the patient frequently and work tirelessly to stay ahead of pain management, and difficult symptoms, as well as help family members, provide the necessary support.
We Offer the Best Care
It is our belief that every patient deserves the best care. AFHC accepts Medicare, Medicaid, Blue Cross Blue Shield of Texas, private insurance, and at times, special circumstance patients. This is one of the most important times in someone’s life, and our compassionate team is here to care for patients and their loved ones every step of the way.

We Provide Our Own Medical Equipment
You have enough on your mind, so we take the stress out of obtaining the medical equipment your loved one needs to remain comfortable. We own our own durable medical equipment company and are able to place and extract equipment quickly and efficiently.

We Will Meet Your Spiritual, Physical, & Emotional Needs
Our team is not limited to medical professionals. We provide each patient and family with the resources and personnel they need to navigate every stage of the hospice experience. This includes:
- Chaplains
Chaplains are here to help patients and families cope with any spiritual concerns at the end of life, either directly or by coordinating services with the patient’s spiritual advisors or personal pastor. - Social Workers
Our team of licensed and certified social workers coordinate resources and assist the patient and family with various non-medical concerns. They can assist family members with the emotional issues that accompany illness. - Bereavement Coordinators
Alinea Provides bereavement coordinators to assist both patients and families deal with grief. Workshops and support groups are also available to provide additional support as needed. - Volunteers
Volunteers are available to provide companionship and emotional support to both the patient and family. They help in a large number of ways from running errands to simply sitting and talking with the patients. - Certified Nursing Assistants
Our nursing assistants provide necessary support to patients and their families. Our CNA team is compassionate and caring. They will work with you to ensure your loved ones remain comfortable and find peace through their hospice journey.
- Board Certified Physicians
Our Physicians coordinate with each patient’s personal physician for treatment management. They will prescribe needed medications and therapies directed to relieve suffering and manage other symptoms of the illness. - Music Therapists
Through every stage of life, music can provide us with comfort and joy. Our music therapists use music as a way to complement our existing services, provide stress relief for both caregivers and patients, and to help with symptom management. - Massage Therapists
Touch is a universal language and provides a number of benefits to hospice patients. Through our massage therapy services, we can help relieve pain, anxiety, nausea, stress, and other conditions. We believe massage therapy provides our patients with a healing presence and can help them achieve a sense of peace and calm in their body. - Licensed Nurses
